When finished, click on the 'Agree' button at the bottom to sign up for Business Online Banking.
Business members may request access to Business Online Banking by completing the Business Online Banking Application Enrollment Form and Disclosure. Forms are available at the FORUM Credit Union website at https://www.forumcu.com/resources/member-forms. Under the Business Category, select Business Online Banking Enrollment to complete your enrollment request. Completed Business Online Banking Enrollment forms will be routed online to the FORUM Commercial Group for processing. Your log in credentials will be securely emailed within (3) business days after receipt by the Commercial Group.
Online Enrollment will not be completed until the Business Online Banking Application Enrollment Form is received. Please call FORUM's Member Service team at (317) 558-6299 with any questions.
